Lake Forest Health & Fitness Center Membership Polices and Procedures
Monthly Dues
All members must provide a form of payment for EFT billing. If the member wishes to pay their monthly dues by another form of payment, the payment must be made prior to the first of the month.
Corporate and Organization Memberships
It is the member’s responsibility to inform LFHFC if they qualify for a Corporate or Organization membership discount and provide proof of qualification. The change in cost of monthly dues will be made effective the first of the following month. The special rate is not retroactive and will not be prorated.
Junior Membership
For those 15 - 22 year olds whose parents are members, and who live in the same household, a Junior Membership option is available.
15 - 17 year olds cannot join LFHFC unless their parents are members, and if the parent cancels, the Junior Membership is also cancelled. The Junior membership rate does not apply if the dependant does not live at home, or has separate billing information.
Student Membership
For those Students attending school locally, the membership rate is $150 to enroll and $78 per month. Students must be at least 18 years old, or if under 18, a parent must be an active member, and they must provide proof of full-time status.
Student Yearly Memberships
Student Yearly Memberships are available for those Students attending school outside a 50-mile radius of LFHFC at a cost of $420 for one calendar year. Student Yearly Members must be 18 years of age or older, or a legal parent or guardian must be a member. A valid school ID or schedule will be required at time of sign-up to purchase a Student Yearly membership. A Health History Questionnaire must be completed and a Physician’s Approval may be required before exercise. Student Yearly Members must provide payment information if they wish to have House-Charging privileges. House Charging on another member’s account is not permitted.
Temporary (Limited) Memberships
Limited Memberships are available for those living outside a 50-mile radius of LFHFC. Completion of a Health History Questionnaire and a valid ID with proof of address is required to purchase a Limited Membership. A Physician’s Approval may be required before exercising at LFHFC. Age restrictions apply (see Guest Policy). Limited Members do not have House Charging privileges, and may not House Charge on a member’s account.
Cancellation
To cancel membership, members must complete a cancellation form with a Membership Advisor at least 30 days prior to cancellation date. Cancellations are effective the last day of a month and cannot be prorated. Cancellations received on or after the 1 st of a month will be processed at the end of the following month. Verbal notice of cancellation, both in person, or by phone, is not valid, and does not stop the accrual of monthly dues. A reinstatement fee is required if you rejoin LFHFC at a later date.
Bridge
A Bridge is offered to hold a membership while a member is unable to use the facility. Bridges are for a minimum of 2 months and a maximum of 6 months, and must start on the first of a month. While on Bridge, the member pays 1/3 their monthly dues, and there is no rejoining fee upon return. If a member wishes to use the club while on Bridge, they can either reactivate their membership and pay for the rest of the month, or pay a half-priced daily fee. If one person on a Couple membership Bridges, the remaining member pays the regular individual dues, and the bridging member pays 33% of the remaining balance.
Medical Hold
A membership can be put on hold at LFHFC for medical reasons with no monthly charge. A note from a Physician is required to put a membership on hold, and the member cannot use the facility while on Medical Hold. Medical holds can be effective for a minimum of 1 month, and a maximum of 6 months. If a hold is to last longer than 6 months, the member is required to renew the hold with an updated Physician’s note. If a member wishes to return to exercise before the end of the Medical Hold, a Physician’s release to exercise is required. The billing freeze with Medical Holds is not retroactive. After one year of being on Medical Hold, memberships will be cancelled. If a member is able to return at a later date, a Physician's Approval will be required, and they will be charge no reinstatement fee.
Guest Policy
LFHFC is a private club. To use the facility as a Guest, you must be sponsored by an active member and pay a $20 Guest Fee. Upon check-in, guests are required to complete a Guest Registration Form. Please note that a Physician’s Approval may be required before exercise. Guests must be 18 years old (15 - 17 if accompanied by a parent or guardian) with a valid ID. Guests do not have access to House Charging on a member’s account.
IHRSA Passports
We welcome IHRSA Passports. IHRSA members who show their home-club passport can use the club for a half-priced daily fee, or purchase a Limited Membership at regular price. IHRSA Passport must be presented upon each entrance into the club when purchasing a Daily Guest Fee. All Guest and Limited Membership policies apply.
Guest Passes
Guest Passes must be validated by a Membership Advisor, and are for one complimentary workout unless otherwise stated on the pass. The Guest Pass must be presented at time of check-in, or payment of the $20 daily fee will be required. Guest passes cannot be used by current members on restricted memberships. This includes but is not limited to: Senior, Matinee, or Weekend Membership Types and/or those members who are on Bridge or Medical Hold. Expired passes will not be accepted. All Guest Policies apply.
